AUTHORIZATION, RELEASE AND WAIVER OF LIABILITY FOR INJURY, USE OF PHOTO AND AGREEMENTS
The safety and well being of you and your child(ren) is our top priority at Amelia Music Studio (AMS), we (Carrie Anderson Mays-business owner, instructors, and volunteers) will not be responsible for and will be forever RELEASED from any injury(ies), claims, and/or any other liability arising from your child’s attendance and participation in our services and programs and hold harmless from any and all injury(ies), claims, causes of action, and/or any other liability arising from your child’s attendance and participation in our services and programs the above-mentioned RELEASES at Amelia Music Studio. You make use of said services. By signing this waiver you affirm that you have decided to allow your child to participate in programs and services. By signing this waiver you affirm that you have decided to allow your child to participate in programs and services provided at Amelia Music Studio with the full knowledge that the RELEASEES will not be liable to anyone for personal injuries and property damage your child or you may suffer while at Amelia Music Studio.

Photos and or videos of the student may be used by AMS for marketing purposes. To remove photo/video permission email: info@ameliamusicstudio.com

Payment for services is due prior-to the services being provided by the first of the month. A late fee of $20 will be assessed if payment is not received by the 7th day of the month. Amelia Music Studio has the right to postpone or reschedule services until payment has been received.

Services canceled 48 hours in advance will be issued a credit for future scheduled services. (Not to exceed once per semester.) Cancelation messages are not accepted outside of the portal.

Services canceled within 48 hours of the scheduled event may be issued a partial credit at the discretion of the owner (e.g., illness, emergency). Cancelation messages are not accepted outside of the portal.

A substitute instructor or a virtual lesson may be used if the scheduled instructor is unavailable.

An administrative fee of $35 is due at the time of registration.

Address concerns or issues which arise with the owner directly (904.535.1372). There will be no posting on social media of any derogatory comments.

A 30 (thirty) day notice (sent to info@ameliamusicstudio.com) is required prior to ending services with AMS. Paid services outside of the 30 day notice will be refunded.

AMS instructors/contractors are prohibited from engaging in services privately with AMS customers until both the instructor (contract terminated) and the customer (date of last service) are no longer working with AMS for a period of one year.

Students are expected to be on time for lessons and committed to practicing at home. Siblings and friends of the student are not allowed to attend the lesson without obtaining prior permission from the owner.
